Responding to "How are you?"

NOTE: this is just a little bit to get it started
In some countries (the United States, among others), this question is often asked as an alternative greeting. In these countries, the expected answer is usually, "I'm fine/good. How are you?" Answering honestly may be responded to with surprise or confusion.
In other countries, this question may be a legitimate inquiry to your health.
